It may be more effective than papain urea at degrading collagen and elas- tin and is thought to work from the bottom of the wound up.33 Research supports collage- nase as a more selective enzyme that may be more effective in controlling or reducing pain when compared with other debridements.29,36 Collagenase up-regulates the migration of keratinocytes over the wound bed and stimu- lates granulation.37,38 Muller and colleagues found debridement with collagenase to be quicker and more cost-effective than autolytic debridement with a hydrocolloid dressing in 24 patients with pressure ulcers.39 Research by Riley and Herman40 has shed new light on the role keratinocytes play in wound healing and how wound healing is influenced by different substances, including enzymes.
